The Health Risk Assessment (HRA) is an online tool provided by Krames StayWell that provides you the opportunity to enter information about your daily living activities, exercise and nutrition habits. The Krames StayWell systems use this information to determine how your lifestyle decisions influence your health, both today and in your future. You are the only one who has access to your customized individual report. Krames StayWell does not record or maintain a database of any Personalized Identifiable Information (PII) provided by you or to you through this on-line tool.

The Krames StayWell Health Risk Assessments do not collect Personalized Identifiable Information (PII) or Personal Health Information (PHI) from customers and/or end-users. In the event our policy changes, Krames StayWell will notify Baylor that the policy has been modified and fully explain the new terms in this Privacy Statement.

The Krames StayWell Health Risk Assessments do utilize passive tracking mechanisms. Krames StayWell will set a cookie on a machine to determine if the end-user should have access to the subscribed Krames StayWell content, determine what content should be displayed in subsequent pages or provide access to specific content areas on Baylors web site. Krames StayWell does not track individual users throughout the site(s) or HRA, or track the result pages.

This tool is hosted on Krames StayWell servers. Periodically, server log files are analyzed using Web Trends via the standard configuration.

Completing an assessment means you are concerned about your well-being and motivated to make the most of your health. The goal for this health survey is to assess your likelihood of developing common medical problems or injuries in the future. Understanding these risks will help you to make active, informed decisions about your health. You will gain a better sense of your overall physical, mental, and emotional health and learn how to improve your health habits. Before you take any action based on the information in the survey, you should discuss the results with your health care provider.

Our health care professional directory may ask you for information on your geographic location or zip code in order to identify listed professionals in your area.

Class Registration and Bill Payment

We offer class registration and payment as well as bill payment. We will request that you provide your name, e-mail address, credit card number and expiration date, phone number and billing and address.

This information will be kept in the strictest confidence. When possible, we will encrypt information for secure transmission over the Internet to maintain the privacy and security of this information at all times.

How We Use Your Personal Information

We continually seek new ways to maintain your trust as a user and improve our offerings. To provide new and better services, we monitor user traffic patterns and try to analyze what our users like and do not like about our current offerings so we can design better services for you. Third-Party Relationships

We work with several types of third-party vendors, most often contracted through HEALTHvision, including those that provide products and services that we integrate to our site, sponsors that provide us revenue through logo and link placement on our site, advertising companies that serve up advertisement programs for the site, and organizations that maintain this site. We request that third-party vendors supply us with information on their security procedures, and we evaluate them periodically to ensure that they are using data in the agreed upon way.
